2007 - Initial exam I was told moderate to severe level. I rejected.
2010 - I had a episode; I was treated with meds and again told moderate to severe indirectly after being subjected to a formal assessment; Understood assessment was used to determine treatment plan.
2011 - January began treatment; records obtained by mental health team - moderate to severe
2011 - July in a 30 minute session with NUMEROUS INTERRUPTIONS determination changed to MILD
2011 - Present: I continue to daily deal with intertwined mental and medical condition. A major question for me is what did I do to deserve the
mistreatment that I receive?

I'm sorry you have been going through so much. I can't speak on behalf of the providers who have seen you before, but I can say that what you describe sounds like at least moderate GAF impairment. You would be justified in seeking a second opinion given the fact that a mild GAF impairment indeed seems inaccurate to me.
