Suggest Treatment For Gestational Diabetes Mellitus

Question: Hi Doctor

My bhabhi is in 28th week of pregnancy. when she went for test her sugar level was around 340. I have the following question

1 what should be done
2 what will be effect on baby and mother
3 should we hospitalize her or wait and watch her situation.
4 what medicine and food you recommend.

since she is located in remote location and has very little access to good medication so I am heavily dependent on your suggestion and guide.

Hello,

Thanks for writing to us. Followings are my comment:

1) According to information, she has developed Gestational Diabetes Mellitus (GDM). In this stage, controlling and monitoring of blood glucose is MANDATORY to prevent maternal and fetal complications.

I suggest you to contact with Obstetrician or admit her to nearby hospital for proper management with INSULIN therapy (BEST) and/ or Oral Metfomin.

She must undergo OGTT (Oral Glucose Tolerance Test), HbA1c, RBS (Random Blood Sugar) periodically following consultation with her doctor and supportive therapy can improve the condition with great extent. One ultrasound scan and periodic assessment of blood pressure is also needed.

2) Maternal and fetal risk is largely related to uncontrolled high blood glucose levels and its consequences.

It includes large (MACROSOMIC- M/C) or small for gestational age, Intrauterine Growth Restriction(IUGR), birth defect etc.

AFTER BIRTH- There is an increased risk of low blood glucose (hypoglycemia), jaundice, high red blood cell mass and low blood calcium & magnesium.

Untreated GDM also interferes with maturation, causing IMMATURE babies prone to respiratory distress syndrome due to incomplete lung maturation.

GDM mother may be associated with PRE-ECLAMPSIA (Pregnancy Induced Hypertension) or ECLAMPSIA (with convulsion).

3) As you mentioned that it is REMOTE area, then it is better to hospitalized her at the earliest for proper treatment. Hopefully, all medicines will be available in the hospital and it will be more effective.

4) Diet control with LOW GLYCEMIC foods are mostly preferred. Her daily diet would be 50% vegetables and fruits. I would give lists of fruits and vegetables which can be taken to reduce blood sugar.

FRUITS: Lemon, Blackberries, Mandarin oranges, Nectarine, Raspberries, Orange, Strawberries, Pear, Honeydew melon, Plum, Cherries, Pomegranate, Figs, Grapes etc. Basically all sour fruits are quite useful in controlling blood sugar. But, it will be taken in adequate amount and not too much.

VEGETABLES: Artichoke, Leeks, Lettuces, Mushrooms, Avocado, Okra, Beans, Onions, Olives, Bean sprouts, Peas, Pickles, Broccoli, Radishes, Brussels sprouts, Sauerkraut, Cabbage, Cauliflower, Celery, Spinach, Collard greens, Eggplant, Endive, Tomatoes, Escarole, Turnip greens, Bell peppers, Water chestnuts, Watercress, Zucchini etc.

OTHER FOODS: 100% whole grain cereals like Oatmeal, Barley, Rye etc; 100% whole wheat floor; Nut and seed milk, seeds, egg, fish, salad, soy beans etc
