Suggest Treatment For HCV

Question: my wife is 34 years old and she is HCV patient, we came to know recently about it

her viral load 477000 ml

log value:- 5.68

1) i wanted to know if her viral infection is high

and wanted to know .how old is virus

she doesn't have any symptom of HCV



2) and is she compatible for IVF treatment or pregnancy



3) from last 1 year we are having sex without any protection and i checked my test report ,it is negative

so i i have sex with her in future with any protection, will i get HCV ?

Detailed Answer:
Hello,
As you have mentioned in the history , lemme first tell you if your Anti HCV test is negative then yes you are not carrying the virus.
Secondly yes your wife s viral load is high, but treatment can be decided only after checking the HCV genotype( there are six genotypes). Treatment options and duration of treatment depends on the genotype of virus she is carrying. For this she need to check her HCV Genotype .
Then depending on her genotype treatment duration such as 24 weeks or 48 weeks can be decided with Pegylated interferon and ribavarin combination. Consulting your Gastroenterologist with regular follow ups and getting the parameters checked to see the treatment response and any side effects of the drugs is very important .
But now a days there are these direct acting agents (DAA) that have come which promise complete eradication of the virus.
Next is that parenteral(mother to child transmission of HCV virus is very less , 5- 6%), Even if she delivers breast feeding also have not shown any transmission of virus unless until there is any crack in the nippel area.
So my suggestion would be consult your Gastroenterologist get her genotype checked, check if DAA are available , as they are the best option and have promising results, otherwise treatment with Peg interferon and ribavarin is also very good, but regular follow ups with frequent testing for response and side eeffects is a must.
