Suggest Treatment For IBS And Urinary And Bowel Incontinence

Question: I am trying to figure out what consultancy service I need. I have 2 problems, both impacting hugely on my life & I need to know where to begin- while both problems are in the same region of the body, they seem to be operating independently , while simultaneously having a knockon impact, one on the other. I was diagnosed with mild IBS about 4 years ago, which is when I can trace the origins of my first problem to. namely, I can get stomach spasms after I eat followed by total evacuation, always diarrhoea. I have had food intollerance tests, tried to stay off but no XXXXXXX I cannot find common pattern or foods when these spasms may/may not occur cept that, if it occurs it is usually 30 mins after food consumption. My 2nd problem in short is that I can't hold it or urine for that matter. I know there is a new pelvic floor centre in Dun Laoighaire but will they address both issues there. All guidance to put me on the right road would be hugely appreciated as things are getting much worse lately & I'm terrified of loss of control in public. I have private insurance- where do I begin, do I need a referral to 2 consultants? XXXX

Hi XXXXXXX
You seem to have to separate problems. The first indicative of IBS ,signs and symptoms that you have being typical of that, is basically a gastro-enterological problem. It should be tackled by a medical gastroenterologist. From the history you give, it appears to be typical of IBs, with lactose and carbohydrate intolerance.
With regard to the other problem, where you have urine incontinence. That will be addressed by a uro-gynaecologist. There are medicines available to control the incontinence. With regard to the centre you named, a pelvic floor centre may possible address the 2nd problem, but maybe not the first. You may very well have to consult separate consultants for both.

Thank you Dr XXXXXXX for that. Is there any consultancy , of which you are aware which can address both problems. I may not have been clear with my initial information but I also have bowel incontinence? And if so can you direct me a particular route?

Well, the bowel incontinence is likely a surgical problem - the gastroenterologist can help with that. You won't find a consultancy which addresses both problems at the same time and place except a teaching hospital. I suggest you make an appointment at the respective departments at a major hospital near you. You could also start with the centre you named earlier to address the urinary incontinence first and then at the hospital for the other problem. I suggest handling them one at a time.
