Suggest Treatment For PACS And PVCS

Question: Hi I have seen a cardiologist and have had all tests on my heart. Everything is normal and I do get some PACS and a few PVCS. Cardiologist says my heart is structurally normal and those beats are not of any importance as my heart is very healthy. He says my palpitations are due to my severe anxiety and health anxiety. I have started seeing a therapist for CBT this week. If I get anxiety under control will the palpitations stop? Are palpitations normal or common with anxiety?

I can understand your concern. As the cardiologist has said that heart is normal so we can easily rule out any cardiac cause for such symptoms. But there are other possible medical reasons that should be ruled out like hyperthyroidism, low vitamin B12 levels etc. Get your B12 and thyroid hormone levels done to rule out these conditions. Hyperthyroid can present with palpitations and other symptoms you are having. Similarly low B12 levels can present with similar symptoms Both conditions can aggravate anxiety also. Start taking B12 supplements and you will see improvement.

If we can rule out medical causes then we can diagnose anxiety as cause for your symptoms. Visit a psychiatrist for confirmation of diagnosis.

Yes control of anxiety will help to stop the palpitations.

Yes palpitations or awareness of heartbeat or rapid heart beat is commonly seen in anxiety and nothing to worry.

Hi Doctor Thank you for your response. I have had my thyroid checked and it came normal. My B12 WAS 176 it was considered in the low range of normal. Is it still too low? Also anxiety can cause the PACS and PVCS?

Hello again

Yes Vitamin B12 levels of 176 are in lower range and as per my opinion these levels are contributory to your symptoms of anxiety. These levels can cause anaemia which may further potentiate anxiety. In various research studies low levels of B12 are found to cause anxiety and symptoms of palpitations.

Consider taking Vitamin B12 containing supplements and also increase the intake of B12 containing foods like animal foods as milk, eggs, curd etc.

Anxiety can cause premature beats and yes it can result in PVCS and PACS. Both can result in palpitations and may appear like anxiety.

So I would advise you to visit a psychiatrist for treatment of these palpitations and anxiety. Relaxation exercises can help to reduce the anxiety. Breathing exercises, Progressive Muscle Relaxation Exercises both are useful in these symptoms. You can take help of YOUTUBE to learn these methods. If symptoms persist even after that then medicines like SSRI drugs can help to control anxiety. Low doses benzodiazepines on SOS basis are also used to control anxiety symptoms.

Any good multivitamin supplement containing B12 would be sufficient.

Hello again

For B12 deficiency you can go for any multivitamin supplement (numerous brands available with chemists) containing Methylcobalamine or Cyanocobalamine. Both are good forms of B12 and will prevent any deficiency. 500 micrograms of Mecobalamine would be sufficient in daily doses and get repeat B12 test done after 12 weeks.
