Brief Answer:
Myomectomy not needed in your case

Detailed Answer:
Hi dear,
I have gone through your questions and understand your concerns.
I have reviewed the reports and the prescription papers.
It shows bilateral polycystic ovaries. PCOS is a hormonal disturbance which leads to polycystic ovaries, irregular or no ovulation and hence infertility.
It do not resolves completely in the long run. Metformin is the drug improves the insulin resistance, helps in improving the hormonal profile and assists conception in PCOS patients.
The follicular study shows dominant follicle in the left ovary, which a sign that ovulation can occur in this ovary by giving hCG hormones, which assists in ovulation of a dominant follicle.
The fibroids in the uterus are very small and not in that location which are likely to affect the chances of conception. In this situation, myomectomy is not needed at all. Unnecessary myomectomy can lead to disruption of the uterine cavity further decreasing any chances of pregnancy.
Conception in PCOS patients can be achieved in a stepwise manner, the subsequent step to be followed when the previous one fails:
1. Ovulation induction with drugs.
2. IUI with Ovulation induction.
3. IVF
I will suggest you to discuss with your doctor the above options along with metformin to help you get pregnant.
Hope you found the answer helpful.
