Suggest Treatment For PCOD In An Overweight Person

Question: Hi Doctor,
Please find attached the report of my wife. She has Hypothyroidism. She is taking tablet Thyronorm 25 mcg (empty stomach in the morning). My queries are following:

1. Can she take any other precaution apart from taking medicine?
2. We are planning pregnancy as well and as per doctor's suggestion TSH level should be upto 2.5 for the same. She has started medicine around 10 days back, after how many days/weekd we should get her TSH levels checked again?
3. She is trying to reduce weight but no results till now. Any suggestions for reducing weight as she has hypothyroidism? Though she is not overweight but she has PCOD so doctor has suggested to reduce weight.

HI and thank you so much for this query.

I am so sorry to hear about this low thyroid state that your wife has. I have reviewed the treatment fully and she is in a state of hypothyroidism as justified by the high TSh. With her on medication, this will for sure come down. I will suggest that we do a control in one month’s time. As for the pregnancy, my advice is that we wait for a oupl elf months, get a stable TSH around 2.5 before continuing with pregnancy plans to be very safe. her weight would also improve on treatment as hypothyroidism is know to cause fluid retention and weight gain. The medication alone is enough and there is no reason to go trying other measures for this disease state.

Thanks for your reply.

One more query....You have suggested that "we do a control in one month’s time". Does that mean we go for her thyroid test after one month of starting medicine and increase or decrease the dosage accordingly?

You are correct. Dosage of drugs would be adjusted according to her symptoms and the level of TSH. Exercise patience and the exact dosage needed to make a difference would be identified and used.
