Suggest Treatment For TB In Uterus

Question: My wife aged 33 years has tb in uterus. She started AKT4 from last 20 days. That time weight was 58 kgs and now it is 60 kgs. Now one doctor told that AKT4 dose not sufficient as per weight and suggest that Rcinex 600mg, combutol 1000mg, macrozide 1000 mg and benadone 20mg. Which treatment to be taken? Is after 20 days medicines can be changed? Xray of lungs not shown TB in lungs. MT test result 14 positive. Please reply.

AKT 4 contains 1 (tab) ethambutol 800 mg, isoniazid 300 mg, 2 (tabs) pyrazinamide 750 mg, and 1 (cap) rifampicin 450 mg. This dosage is sufficient for a 60 kg person. The increase in dosage is not needed for uterine TB.
After 20 days the doses can be increased but that is not needed in your wife's case. In case of uterine TB X-ray chest does not show any changes. You can get The QuantiFERON-TB Gold test (QFT-G) done for further confirmation.

Thanks for early reply. Hereby I give the facts that my wife not conceived for second baby so Gynaecologist advised the tests as HSG Xray =Left distal (fallopian) tubal block, Mantoux test =14mm and ESR=34. Based on that she diagnosed that TB in uterus. However Chest Xray results normal. She advised that AKT 4 contains rifampicin 450 mg (before breakfast), 2 (tabs) pyrazinamide 750 mg (one before lunch and one after lunch), 1 (tab) ethambutol 800 mg, isoniazid 300 mg(after dinner) and Liv L1 syrup and Rantac (gastic tablet) tablet before breakfast. After 15 days LFT test done which was normal and advised to continue same treatment.
Afterwards when we consult TB specialist then he advised that AKT4 is not sufficient for the person having weight more than 50kgs. He suggested that Rcinex 600mg (before breakfast), combutol 1000mg & macrozide 1000 mg (after breakfast, i.e. all tb tablets with in two hours) and benadone 20mg. As per him these are modern and effective tablets and AKT4 tablets banned outside india.
Hence please advice that (1) which medicines to be taken to cure TB in better way
(2) What timings to be preferred and how important timings of medicines in TB.
(3) Whether AKT4 tablets cured TB 100% and if AKT4 (instead of separate tablets suggested as above) continue than what difference of treatment of TB.
(4) Since 20 days completed if now medicines or tablets changed than what effect like drug resistant TB. Actually I heard that if timings or medicines changed than it become drug resistant TB
(5)Whether it is TB infection or TB disease and if infection only then also 6-9 months medicines required?
(6) Whether any other test required or medicines can be continued
(7)How much time TB medicines to be taken and whether in this period treatment for tubal blockage required.
(8) Whether tubal blockage removed due to medicines or what treatment required
(9) What status of TB now in my wife’s case
(10) Any other advice which you want to give.

Hello.
Thanks for writing again.
1. The higher doses as suggested by TB specialist will help in an earlier recovery.
2. Timings are important for proper drug absorption and optimum effect. It is best to take all the medicines within 2 hours.
3. 100% cure is not assured for any disease. If separate tablets are taken then the efficacy of the treatment will be better. AKT4 is also equally valid treatment for TB.
4. Changing the doses only will not cause any drug resistance. If you stop the treatment before proper 6 months then drug resistance TB is likely.
5. Complete treatment is required even if it is an infection or a disease.
6. The medicines can be continued and get LFT evaluated every month to see the effect of medicines on liver.
7. It is continued for 6 months. Tubal blockage is removed surgically after the treatment is complete.
8. Surgical treatment. Adhesions are removed to make the tubes patent.
9. She is infected and needs treatment.
10 . You can switch to higher doses and take all the tablets in the morning. In case there is severe nausea or fever like sensation then she can resume AKT4 kit and spread the doses throughout the day.
