Hi,
I had a look at the report you have attached. The small stones in kidney are by themselves unlikely to cause acute pyelonephritis unless one if the had moved down the ureter and caused obstruction which was complicated by UTI and therefore pyelonephritis.
The CT scan you have uploaded is over one month old. If fever or loin pain are persiting, I would suggest she gets a repeat ultrasound scan of KUB done. Check CUE, Serum Creatinine, CBP too. If there is no evidence of serious infection or obstruction, she can try medical therapy for the small stones while being on a low dose antibiotic like Levofloxacin 250mg once daily at bedtime.
If there is swelling of kidney, she might need intervention like a stent.
