Question: Dear Doctor,

My doctor said I have AUB, UTI. I need second opinion please.

I am experiencing blood during intercourse but not always. This starts May 2014-present. FYI: I was taking contraceptive pills from May-July.

By July 2014, I consulted with my OB GYNE. She did a pap smear and the result is normal. Though she thinks that the blood I am experiencing during intercourse might have been caused from the 10 o'clock location on my cervix. It bleeds easily. As a treatment, she gave me Candid V6.

I went back to her August because I still experience blood during intercourse. Hence, she gave me again Candid V6, because on my cervix she saw that the 10 o'clock is gone but now it is the 6 o'clock on my cervix that bleeds easily. Also, she has been asking me to take folic acid supplements and fish oil. I am done with HPV vaccine last 2012.

Aside from this, I told her that I am experiencing small external hemorrhoids due to constipation. She gave me faktu suppository.

September, I went back to her because I still experience blood during intercourse after using Candid V6, two times. This time she asked me to undergo TVS, Urinalysis & CBC (Blood Test). I have attached the results.

Here is my record:

Month of AUGUST

Aug. 10 - 1st day of menstruation. No intercourse.

Aug. 26 - Ovulation. Had intercourse. Withdrawal method. No blood seen, no smell.

Aug. 30 -‹Had intercourse. My partner cummed inside. No blood seen, no smell.

Month of SEPTEMBER

Sept. 4 - Had intercourse. Foul smell (smells like a rotten fish). With blood.

Sept. 5‹- Had intercourse. My partner cummed inside. No blood seen, no smell.

Sept. 6 - Had intercourse. Foul smell (smells like a rotten fish). With blood.

Sept. 8 - No intercourse. Spotting. Smelly. Strong smell (smells like a rotten fish).

Sept. 9 -No intercourse. Spotting. Smelly. Strong smell (smells like a rotten fish).

Sept. 10 - 1st day of menstruation. No intercourse.

Sept. 12 - End of menstruation. No intercourse.

Sept. 13 - Spotting. Smells like old blood.

Sept. 14 - Spotting. Smells like old blood. Pain on the upper right corner of my lower abdomen. On-Off, shooting pain. When it pains, the pain goes until my vagina. I feel like that.

Sept. 15 - Very minimal spotting. Smell lessen.

Sept. 16 - No spotting. Brown color discharge. Smell lessen.

Sept. 17 -‹No spotting. Brown color discharge. Smell lessen.

Sept. 18 - Had intercourse. No blood. Little smell.

Can you please help me? Based on my record stated above and the attached results, what is my health condition? What should I do?

After reading all your history and evaluating your reports I want you to get a colposcopic examination done so that exact status of cervical microvasculature can be assessed properly.
It is not a good sign to bleed after sex each time along with a foul smell.
Get the colposcopy done and then we can discuss the reports.

Hi doctor! Thank you for youe response. Colposcopy? I think it is done when pap test is abnormal. My pap test is normal and I had HPV vaccine last 2012. I'm confused.

I'm really impressed with your knowledge and yes most common indication for colposcopy is abnormal pap smear but in cases with strong clinical suspicion on the grounds of history and examination then colposcopy is the most conclusive test.
Taking the HPV vaccine was a really wise decision but still it will protect you from 2012 onwards but indwelling infection (if at all) would have had done the damage by then.

Do you think this is bacterial vaginosis? I get a foul rotter fish like smell days before my menstruation. Also, bleeding during sex is not always. It occurs also days before my menstruation. Please enlighten me more. Thank you!

Hi no in bacterial vaginosis firstly contact bleeding is not very common and if occurs then it is seen usually on the tip of penis as a fresh small specs of red blood.
Brownish spotting or spotting from vagina after intercourse is not consistent with bacterial vaginosis.
Get the colposcopy done after that take a full course of antibiotics with metronidazole and doxycycline so as to eliminate infection as a whole but in any case colposcopy is a must according to international guidelines.
