Suggest Treatment For A DVT Patient
Question: I am a 55y/o female. I hade shoulder surgery on 4/5/2017. On 4/6/2917 I was admitted for what they thought was pneumonia; on 4/7/2017 I was d/c'd w/a diagnosis of bronchitis. On 4/17/2017 I went to the ER with severe pain in the calf muscle of my right leg. My d-diner was 2.72 and ultrasound was positive for a DVT. I was sent home with oral Coumadin (5mg) subq and Lovenox 0.8mg subs bid. Today I woke up short of breath and dizzy/light headed so I went to the ER per discharge instructions from 4/17/2017. A CT w/contrast was done and did show a PE. Treatment w/the blood thinners was not changed. Also, my blood sugar was 450 when my label were drawn. (I was somewhat dehydrated upon arrival and an IV of NS was started.). After 10 units of insulin and more IV fluids, my bs was 350,and I was given another 10units of insulin. After more time and fluids 3L NS, my bs dipped to 274. My IV was d/c'd, given an Rex for metformin SA and released to go home.
Is this the proper protocol/treatment for a patient with a DVT then a PE 3 days later to go home???? Please answer soon as I am patiently waiting an answer. Thanks-------

Yes a patient with PE is kept under observation in ICU for at least 72 hours before discharge on blood thinners. However once the parameter returns to normal and breathing difficulty is settles down patient is discharged with on oral anti coagulants.
Treatment of DVT is use of anticoagulants for long time.
