Suggest Treatment For A Breast Abscess

Question: My sister has a small circle under her breast that has an outside white circle around it and a scab in the center of it. Should she have a mammogram right away? Is this possible breast cancer or early stages? Or could this be shingles? Her husband just lost his job yesterday.

Dear Ms XXXXXXX

Thanks for providing clinical picture. If the history is of short duration, it is likely to be superficial breast abscess. Scab and surrounding erythema(suggestive of inflammation) suggest the possibility of inflammatory swelling.Its not shingles, as shingles usually has multiple crops of erruptions and they are very painful. Exact diagnosis will however depend on clinical examination.

I would suggest to consult primary care physician for further assessment. In the meantime I would suggest the following

1. Local application of Antibiotic Ointment Like Mupirocin which is over the counter medication

2. Oral Antibiotics like Amoxicillin+Clavulinic acid(Augmentin ) twice a day for five days. If you are unable to consult the Physician , you can use over the counter available antibiotics.

3. Oral anti-inflammatory like Ibuprofen will be helpful.

If the lesion does not decrease in size even after 3 to 4 days of antibiotics, it will require incision and drainage based on clinical finding.

Dr. XXXXXXX I was just wanting to update you per your request. My sister's doctor agreed with your diagnosis exactly. He took a culture by taking a sample from the infected area and has put my sister on an antibiotic in the meantime. Thank you so much again; your excellent expertise is much appreciated.
