Likely to be lymph node
Detailed Answer:
Hi
Welcome to Healthcaremagic
Greetings of the day
Dear Mr XXXXXXX
One of the most possible cause and commonest cause for ganth or swelling in mandibular region is Lymph node enlargement. The history of decrease and increase in size of swelling makes the possibility of swelling to be lymph node more likely.
2. As it responds to antibiotics (the size decreasing ) makes it more likely to be Inflammation /infection ad the likely cause . The source of infection could possibly be Tonsil
3. The chances are it's related to Tonsil based on location especially if it's near angle of mandible (just below the ear) . Exact cause can only be attributed by clinical examination.
4. Before resorting to any test,it requires clinical examination by either Pediatrician or Pediatric Surgeon
further management will depend on it.
5. Weather also has role to play . There is more chance of Tonsillitis especially during rainy and winter season. Exposure to allergen and pollution could possibly be the cause too
6. It does not lead to pain. Pain in ear could be due to Eustachian tube block due to respiratory infection. Assessment by ENT surgeon is required
Do get back to me if you need any further assistance, will be glad to assist you.
Take care

Detailed Answer:
Dear XXXXXXX
As previously advised it's most likely to be Lymph node. Confirmation requires physical examination. Do visit Pediatrician and kindly let me know opinion based on his examination. Will guide you further regarding treatment options based on examination finding and advise of Pediatrician.

If my son does not start the treatment for this ganth which is small now [and which is under his lower jaw not exactly below the ear] then this ganth will grow in size and there after pain will start even touching on it and thereafter fever/cough will start so what is the reason for it provide details around it please. He had undergone the following treatment last time for 5 days after examined by pediatrician:
mefkind p---7.5 ml 3 times daily
moxclav 228.5---3 times daily
Therefore this time i need your advice on line of treatment.
No further intervention required
Detailed Answer:
Dear Mr XXXXXXX
Going by your description the cause of ganth/swelling is likely to be non specific enlargement associated with Viral fever/respiratory tract infection. Reduction in size with antibiotics is reassuring sign and requires no further intervention.
