Suggest Treatment For A Single Isolated Tumor Cell In A Lymph Node
My name is XXXX I am also a R.N. for 30 years. I have a major problem and I need help
I was diagnosed with endometrial cancer well differentiated Stage 1A. I have my post pathology report and myentire chart copy, my Figo grade is 1, I had less than 50% myometrial invasion 2mm/14mm in area of maximum tumor invasion , I had melf invasion. I had no lymphovascular space invasion, I had a Davinci done at XXXXXXX Kettering in NYC. They only took the sentinel lymph nodes My right was benign but the left had a single isolated tumor cell. Now I have a major problem My surgeon at XXXXXXX told me to get full pelvic radiation the medical oncologist said just brachytherapy. The radiation oncologist said it is up to me. I feel as a nurse but I am of course not a doctor that we did not take out enough lymph nodes I
also know full pelvic radiation with irradicate any tumor cells if they are in other lymph nodes. Full pelvic radiation does and can have long term side effects. I have a family I want to live even if it means side effects. I was a very healthy vibrant beautiful girl before this no I am so stressed out. Also I am concerned that the davinci hysterectomy was alot of manipulation of surgical instruments. Well anyhow now today the surgeon calles who is in charge of the case and said she wants me to seek a second opinion because doctors can not agree on which type of radiation. I am a very meticulous person and actually I work as a nursing supervsior of 25 nurses but now I can not even fiqure out my problem. Of course I am on a long personal leave of absence. If I do full pelvic and have a recurrence can I do radiation again or is it just brachytherapy I can follow with radiation They also put me on Letrozole 2.5mg every day of course I am taking it. The oncologist said my P53 was normal and there is different types of melf invasion. When I read melf it said predictor for lymph node metastasis. In your opinion do you think anything could be in other pelvic lymph nodes or we just don't know? i realize I am asking alot of questions and I am more than happy to pay you fully for help Thank you i will wait patiently.

If a single isolated tumor cell has been detected in a single lymph node then it is best to get a full pelvic radiation. Although we know that it is associated with more long term side effects but it will decrease the chances of recurrence or distant metastasis considerably.
If you go for brachytherapy then in case of recurrence you will always have a guilty feeling that radical pelvic radiation should have been done.
