Suggest Treatment For Abacterial Cystitis

Question: My sister went to the doc for what she thought was a urinary infection..they did a urine sample and it came back moderate blood and few WBC..she was given antibiotics amoxicillin and was told they would do a culture and call her back..she started the antibiotics started feeling a little better but still has some pressure in the ureatha..she was called and told the culture came back negative so she stopped taking the antibiotics and returned to do another urine sample to see if the blood was gone. At that time it showed the blood was gone but she still had few WBC, small amt of glucose and specific gravity was a little off. She started having symptoms again pressure..urgency to go and soreness on right lower abdomen. She went back to the doc and they found trace of blood and few WBC again so now the doc put her on bacterium ..what could this be..why did the culture show no infection...and she still has symptoms...please advice

Thanks for your query,based on the facts that you have posted it appears that your sister has what is called as Abacterial Cystitis with Over Active Bladder.

I shall be happy if you can post me few more details to help me in making comments on your problem.
1) What is the age of your Sister?
2) Is she in peri menopausal age?

Absence of any positive evidence of infection with symptoms typical of UTI is mostly due to what is called as Abacterial Cystitis with Over Active Bladder Syndrome .

She needs to take urinary antiseptic like Nitrofurantoin twice daily along with anti muscuranic drug like Darifenacin twice daily.

Get her Ultrasound Scanning of the abdomen done to rule out presence of urinary stone .

Ensure she drinks more water. This will help to keep your dilute .

Now she says that she has found out where the bleeding and pressure is coming from..after a hysterectomy three years ago she developed granulation tissue around the cervix removal site..it continued to bleed for weeks ..she had several silver nitrate treatments..she thought it was gone when she noticed the bleeding was coming from the same granulation site..so she needs to go see her obgyn..that seems to explain the bleeding..wouldn't it? Would thT be picked up in the urine analysis?? The pressure was always deep in the vagina canal..my sister is 55 and she is post menopausal..does this sound logical? Would the open wound from the granulation cause a bacterial infection in the canal. Can the open wound cause infection Thanks so much! What would cause the granulation tissue not heal..bad sutures?

Thanks fo feedback and highlighting the facts .

It is most likely that her urine that was sent for analysis is mixed with blood from granulation tissue.- hence reported in urine analysis.However presence of W.B.C suggests infection hence she needs to continue antibiotics for couple of days.

As regards granulation tissue even 3 years after surgery indicates she has chronic granuloma which needs to be fulgurated completely under anesthesia.

Is the infection due to a vaginal bacteria from the granulation..so you are saying she will need to have surgery for the granulation ..is it an easy procedure..will it cause a lot of problems or infections if she leaves it alone..is the granulation caused from bad sutures..will it heal after the doc removes the tissue or will it create more scar tissue for future grandulation. Thanks so much!!!

Yes chronic granuloma can be a source of infection .

To fulgurate the granuloma is a simple procedure done as a out patient care procedure and there are very little chances of recurrence.It is not related to suture material used .
