Brief Answer:
Commonest cause is sigmoid volvulus.

Detailed Answer:
Hi.
Thanks for your query.
To recapitulate: Male/33 - suffering for 4 years - sudden pain lower left followed by stomach feeling of off balance in feet - no smoking or drinking - eat lots of junk food.

The commonest cause is as follows:
Sigmoid volvulus.
Rarely can be due problem in the blood supply of the intestines.

If you get bloating, that is distension of abdomen which is relieved by passing of lots of gas, it is the time to take one X-ray of the abdomen in standing position to get a proper diagnosis and plan a treatment accordingly.

If the causes are thought to be different you have to undergo further tests of blood, stool, urine, ultrasonography and may be contrast enhanced CT scan of the abdomen. Colonoscopy will help more.
Surely an opinion of a Gastroenterologist will help to get a correct diagnosis.
or still better will be General Surgeon or a Surgical Gastroenterologist.

I hope this answer helps you, please feel free to ask for further relevant queries if you feel that there is a gap of communication.
