Suggest Treatment For Abdominal Pain Associated Bloating, Gas And Nausea

Question: Hi, I recently have been experiencing some abdominal pains associated with bloating, gas, occassional nausea and recently rib cage pain. I sometimes have diarrehea as well and occassional feelings of constipation but not serious there. My pain in the abdominal area started in the lower part and its a shooting sharp pain that happens for minutes and usually goes away very soon now its a little higher up. It started 7 or 8 months ago and went away for a few months and happens intermittently a few days a week. I also am experiencing difficulty sleeping and feelings of exhaustion and fatigue which has been a chronic thing for me but never something I thought much of. I am only able to sleep a few hours at a time never constant and this might have to do with anxiety. My stomach pain is relieved with passing gas but the bloating is still there. The pain in the rib cage area is more like constant feeling of soreness and not really pain and its not something that I cant tolerate just discomfort more than anything but it also comes and goes. Also very occassionally I will have sudden shooting very sharp rectal pains almost like a spasm that lasts for seconds and comes occassionally but goes away within seconds. Also experiencing occasional burning sensation like heart burn. Have difficulty paying attention to stuff and cant focus on one thing as much. Could it be fibromyalgia? I have been reading about that and I seem to fit with the symptoms. I dont have a family history of cancer or heart disease so I am not sure if that would be a possibility. The last symptoms I have are clogged sinuses and a cough not too bad but it feels like I always have a cold and some weakness in my knees and inner thigh and pelvic area. Believe it or not even though there are many symptoms I am still able to do everything normally and they always come separately not together.

Well, XXXX the symptoms you have been describing classically fits into Irritable Bowel Syndrome. It is a disorder of the motility of intestines which can be accompanied with either constipation( IBS-C) or Diarrhea( IBS-D). The symptoms of getting relieved with passage of gas, intermittent abdominal pain do fit into it and it tends to get aggravated with less sleep and more stress.

Considering your family history of malignancy i would suggest you to get a Colonoscopy done once, just to rule out Inflammatory bowel disease which is a very close mimic of Irritable Bowel syndrome as the management differs for both.

But yes Irritable bowel Syndrome patients do have a high incidence of Fibromyalgia( almost 60-70%) which accompanies fatigue, irritability and increased tendency to forget things.

But these 2 are the last diagnosis, that is the diagnosis of exclusion when we have ruled out everything else.

SO now, there are 2 ways to handle it: Either you can get colonoscopy done and a few blood tests to rule out Inflammatory bowel disease or You can try a different treatment for 2-3 months and see if it works.

Among the different treatment, the key is to regulate your sleep and include high fiber foods in your diet. You need to do some exercises and take less of stress. You can try Duloxetine for a while if the symptoms are too bad. But yes, little life style modification will help you.
