Suggest Treatment For Abscess On The Inner Thigh Near The Groin Area

Question: I have a cist/abscess in the forming on the inner left thigh near thigh joint in the scrotal / groin area since four days . Six months earlier I had an abscess on the right side of the groin area which didn't respond to bactopad tab and yet another antibiotic & later 3 oframox injections too. I had to undergo minor surgery with a 4" incision from lower abdomen to just near testis after a week to 10 days initial treatment. I am a diabetic for over 30 years; on insulin for last 3 years; taking tabs for HT as well as heart for nearly 10 years. It took nearly a month for the incision to completely close & heal.

Now for the present predicament, I started taking Ampilox 500 thrice a day in empty stomach & 2 -3hr after meal since 3 days. (8 caps so far). Was applying Mupirocin ointment on the affected area. Now changed to Nadoxin cream as per advice of my doc today. He approved my usage of Ampilox and prescribed Lyser forte 1-0-1 and asked me to continue the medication for another 4 days till the next review. As I don't want to go through the ordeal of another surgery I seek your advice in the matter. (I have a photo of the affected area. How to send it to you?) Is there any way this abscess will subside by itself with this medication in the absence of any culture or sensitivity tests? Is there a way of finding a suitable antibiotic that will have a definitive action?

To recapitulate: Male/66 - known diabetic 30 year - on Insulin 3 years - medication for hypertension and heart for 10 years - abscess on right groin 6 months ago - needed surgery - took a month - now cyst or an abscess left thigh-grin area for 4 days - on Ampilox, Lyser forte started - locally Mupirocin and Nadoxin now - photo - seen - wants to know whether this will subside with medication in the absence of c/s reports - any way to find the suitable antibiotic that will have definitive action...

My thoughts:
Please attach very clear and focused photos, it helps sometimes for on-the-spot-diagnosis.
High resolution ultrasound to see whether this is cyst, infected cyst or an abscess. If liquefaction is present one can go for aspiration as a treatment and to get the material for gram staining, culture and sensitivity which will be most beneficial.
Lyser forte to be taken three times a day.
Continue Ampilox 2 caps thrice daily as it contains 250 each of Ampicillin and Cloxacillin, if you weigh more than 50 kg.
Apply Liquid Betadine locally 3-4 times a day as it keeps the area sterile, not allowing secondary infection.
Take multivitamins as you are diabetic
Daily blood sugar and blood pressure charting and to have both the things in well under control. Control of diabetes is very important.
Tests of blood for CBC, kidney functions.
Tests of urine.
