Suggest Treatment For Acute Renal Failure
Question: Hello doctor,
Is it possible to go into acute renal failure without any risk factors or medication issue? Two weeks ago, I went to ER thinking I had lowered urine output but all my heart, kidney, and liver, and urine tests were normal. I had a prior pvc-induced cardiomyopathy and I am always terrified of a recurrence, presenting with Lowered urine output. I had a doctor who kept telling me before my curative ablation that I would always be in grave danger of CHF so I'm constantly vigilant about my fluid intake and so scared of retaining water and having my heart weaken again.
Is this scenario impossible with my perfectly normal labs and testing? Is this simply my anxiety? I am terrified after every drink I take, afraid I will not be able to process the water.

In the background of all your tests results being normal, I dont think you need to worry about water retention. You have a body which is in perfect balance and I dont think its possible for someone to go into renal failure without a strong reason.

This stems from the hospitalist telling me water was "toxic" for me and that it would kill me if I drank more than just a tiny bit. Even though cardiomyopathy reversed, I am constantly afraid it has/will come back or that my kidneys were affected and I will suddenly be unable to process any fluids and go into CHF. Is this completely irrational? I don't know what will help my fears. Without extremely frequent testing, I am just so scared.

I think it is irrational to have thoughts that way when your doctor has asked you to follow a normal water intake.
Did you get to speak to your psychiatrist about the OCD like I had asked you to previously?

Thank you doctor. Yes I am seeing an OCD specialist about my issues. I keep thinking that only medical tests can reassure me that I am ok because I didn't feel sick when I was diagnosed.
I am working with my psychiatrist and my therapist and hope to see progress very soon.
So you don't believe I need frequent testing or to monitor fluid input and output and to weigh myself multiple times a day?

Dear Dr,
So would it be impossible to be in kidney failure today with lowered urine output after having absolutely normal testing just over three weeks ago?
I am in therapy, on anxiety meds, but I am having problems forgetting the horrifying and scary things that were said to me about dying very suddenly and about water being like poison to me.

If there are no precipitating events like dehydration, loose stools, vomiting, etc you are unlikely to be in renal failure.
I recommend that you stop monitoring your output. Urine output is highly variable on a day to day basis even in normal individuals.
