I presume your sister has adenocarcinoma of uterus. It must have extended to cevix ( neck of womb which she is feeling) and pelvic area. The cancer lesions are very vascular with fragile vessels in some areas. Inadvertent touching/rubbing these lesion can initiate the bleeding. The reason she is not bleeding now could be because during examination these fragile regions are not touched or they have been covered by less vascular tissue.
may be during previous examinations the doctors took biopsy for examinations which initiated bleeding.
The second reason is cancer lesions bleed spontaneously too without anybody touching them. The bleeding she had during her previous examinations could be coincidental.
Has she had radiotherapy? Radiotherapy can cause fibrosis of vascular cancer tissue reducing the possibility of bleeding.

You can get rid of the smell by removing the growth.
The right person to tell whether surgery or radiotherapy is appropriate for her is her treating oncologist after assessing the stage of cancer and her general condition.
