Suggest Treatment For Anal Fissure And Blood In Stool

Question: Two previous doctors advised me that I may have an anal fissure, but I'm not certain I gave enough information, because from my own reading that doesn't totally fit my symptoms.

About two weeks ago, I experienced some rather severe rectal discomfort/pain. Bowel movements were normal, no blood in stool but some on toilet tissue. The discomfort gradually subsided over the course of a few days to the point now of being very moderate. But there is still some rectal tenderness and discomfort, as well as some blood, again not in stool but on the toilet paper, as well as swollen area around the anal base. Also, what I can only describe as small stool buildup over the course of the day around the rectum -- not discharge but buildup. I've noticed being in a hot shower noticeably relieves the symptoms.

My understanding is that a fissure is very painful, but except for the first two or three days, I've experienced little real pain. Would hemorrhoids be a more a likely cause? I know they can also be painful, but again, pain isn't one of my main symptoms.

As per your description, symptoms have started recently.In haemorrhoids, in early stages,bleeding is the only symptom.Blood will fall as drops.In late stages,when the haemorrhoids get thrombosed, there will be pain.
A bulge that is palpable could be a skin tag due to fissure in ano.Pain will be mild in few and severe in others.
Your symptoms goes more in favour of Fissure in ano.
Please take plenty of liquids, eat high fiber diet .
Your stool should be soft.
Treatment will be same for Fissure in ano & early haemorrhoids.

Thank you so much for your helpful answer. I was mildly concerned these symptoms could indicate something more serious, such as anal cancer. But your information helps to ease my mind in that regard.

I prescribe Ointment containing Diltiazem.

Apart from dietary modifications, I prescribe Ointment containing Diltiazem to my patients with anal fissure.Response was good.
There is another cream which contains Sucralfate and Lignocaine which works good in few patients.

You've been very helpful, sir. May I ask one final question?

My only remaining concern is that so many people seem to say that fissures are very painful. Of course I don't want to be in pain, but the fact that I've experienced more discomfort and not so much outright pain made me wonder exactly what was wrong. But it's sometimes the case that fissures are not as painful for some people as others?

Fissure is nothing but a crack in the sensitive anal mucosa.Once the stool touches the fissure,the pain starts depending on the hardness.If the stool is soft,there will be minimal pain.
So,your stool should be soft for minimal discomfort.
