Brief Answer:
Try Dilitazem or go in for Surgery.

Detailed Answer:
Hi XXXXXXX What allopathy have you tried? Dilitazem ointment? If no treatment gives relief then there is a high chance that her sphincter pressure may be high leading to spasm of muscle during passing motions. If Dilitazem does not work then I would suggest surgery in which the internal sphincter is divided to relieve the spasm. This has good results and the pain will go away. It is a small surgery with hardly any complications. You should discuss with your Surgeon who can explain it to you. Its called Lateral Internal Sphincterotomy. You can any doubts you have about it. Take care.

Thanks for your reply..the doctor suggested THANK OD LD cream but is not working out in reducing pain. also there is a syrup called DUPBALAC oral solution and GUTFEEL powder which is helping in smooth motion.

Is Wheat germ oil and aloe vera gel good to reduce pain??

Brief Answer:
Use Diltiazem ointment or opt for Surgery.

Detailed Answer:
Hi. The cream is usually used for Hemorrhoids or piles. I dont use it for fissures. The syrup and powder are to soften stools so they are alright. Aloe Vera and wheat germ can reduce pain in some patients but not all and have a temporary effect. Surgery is safe and complete treatment. For patients who want to avoid surgery I usually advice Diltiazem ointment for local application mixed with Lignocaine gel twice a day. This decreases sphincter pressure and reduces pain. Do let me know if there are other queries. If not please help us by rating the answer. Take care.

Brief Answer:
No substitutes. Try for a month.

Detailed Answer:
Hi XXXXXXX They should be used about half hour before passing stools so that the muscles relax. Lignocaine plain can be applied even after passing to make the area pain free if required. There are no substitutes- these are chemical names and can be found in market under different brands. Diltiazem should be applied lying down as it can cause headache. Both to be applied little inside and little outside. Should consult your Doctor if she has Hypertension. You should give a trial of a month for both the medicines. Take care.

All the drugs mentioned above are prescription drugs, and needs to be taken under the supervision of your doctor.
