Question: hello doctor !! i just need a help with something happening to me from last 4 days

Hello,
the pain you are describing is like that seen in an anal fissure.
I would advise the following

A stoolnsoftner like psyllium husk which will make the stool consistency softer and reduce pain while passing stools

Xylocaine point ment to be applied locally beforeassing stools will help in reducing the pain.

Sitz bath sitting in a tub of lukewarm water twice a day for 10 min. Continue this for 5 days.

How ever I would ask you to vist your doctor and get a rectal exam to confirm the diagnosis.If the anal spasm is very strong he might prescribe local diltiazem ointment

and there is a kind of wound in my anal region with a little swelling in the middle . is that also symptoms of anal fissure ??

It could be an peri anal skin tag or piles. We also need an exam to rule out an abscess
