Question: i am suffering from Fissure at 3 O clock , suggest medicine and ointment . Also diet details .

Following are the predisposing factors for occurrence of anal fissure

1. Decreased intake of fiber and fluid leading to constipation

2. Stress

I hope the diagnosis has been confirmed by your Physician.


I would suggest you the following

1. Ensure good intake of water : 2 to 3 liter a day

2. High fiber diet : 2 servings of vegetable salad and 2 servings of fruits especially citrus fruit will ensure adequate intake of fiber in diet.

3. Stool softner to ensure soft stools which helps relive constipation ::Laxatives like Lactulose will be helpful

4. Local application of Xylocaine ointment will help relieve pain .

5. Nifedepine ointment will help in resolution of anal fissure . It requires prescription from registered medical practitioner

Thank you for your reply.

As I understand ,

1) should I continue to take 1-0-1 following medicine for 7 days as
ciprofloxacin 500mg Rantidine 150mg Polyvitamine tab Diclofenac sodium 50mg

2) Use Xylocaine ointment in stead of Anobliss ointment ?? Use before and after bowl...??

3) Citrus fruit such as .....?? Orange , Moasambi ??

4) Lactulose intake daily 1-01- ??

Kindly inform on above my query.

Dear Sir

Let me reply to your queries.

1. Antibiotics are not indicated in management of anal fissure alone. So there is no need of taking Ciprofloxacin.
You can continue taking Multivitamin, antacid like Ranitidine and analgesic like Diclofenac for effective control of pain


2. Use Xylocaine ointment instead of anobliss , use it 15 minutes before passing stools and 15 minutes after passing stools.
Use Hot water Seitz bath : To a tub of lukewarm water add 2 spoons of antiseptic solution. Sit in it for 15 to 20 minutes each session twice a day. It will also help relive pain and muscle spasm

Also apply Nifedepine ointment twice a day , 2nd application just before retiring to bed.

3. Citrus fruits like oranges and moosambi . Also do eat vegetable salad like cucumber, beetroot, carrot etc

4. Syrup Lactulose 10 ml at night during bedtime should be sufficient to ensure soft stools
