Question: Hello doctor I am 25 year old male I am suffering from obsessive thoughts and social anxiety unable to control it and always day dreaming about something all day long, not able to concentrate in my work or anything. I am suffering from this problem since childhood and always failed in school and college. my cognitive ability is weak I have problem in learning and thinking. I have heard about smart drugs like nootropic for cognitive development is it safe to take it or can you suggest any medication, so please help me otherwise my life will be gone.

I understand that you are distressed by your long-standing psychological problems. From your description I can understand that you are mainly suffering from lack of concentration, anxiety and obsessive thoughts. You have also mentioned that your cognitive ability is weak.

Now, it is well known that anxiety and obsessive thoughts can result in poor concentration and poor cognitive performance. So, it would be worthwhile to try medication which can decrease your anxiety and negative thinking, so that your concentration and thinking process is clearer. But, before that, I would advise you to have a detailed psychological evaluation by a psychiatrist. This evaluation would include a clinical assessment as well as certain psychological tests, including an IQ test.

In your current scenario, I would recommend you to try an SSRI medication like Escitalopram - which is very effective for controlling anxiety and obsessive / negative thinking, and thereby improving your concentration and cognitive performance.

Nootropics or cognitive enhancers don't have a proven efficacy in all persons with cognitive problems. They are usually used only in cases where there are neurological problems causing the cognitive deficit.
