Suggest Treatment For Anxiety, Worthlessness, Decreased Appetite And Death Wishes

Posted on Sat, 24 Jan 2015
Question: I have lately been very worried about my time on Earth. I have been been feeling like a loser lately (eventhough I'm doing fairly well. I just think I could be doing better.) I worry what I am going to do if my parents pass. My sister passed away 4 years ago on Christmas. I've always loved Christmas but this year I have been feeling very sad. I can't seem to shake it. I am single and I feel that I'm going to be alone sometime. I'm not eating and I feeling very worried. I don't understand why I feel this way this year - not 4 years ago. I am going to go on a 1-week trip soon and I'm really worried to leave my parents alone. They are doing well - just getting older. 77 and 78. They don't do a lot of socializing and I worry about them when I'm not home. I have an adopted son who turned 19 recently (and is doing more things on his own). I noticed that watching movies has given me short periods of time when I feel ok. Should I go see a therapist or can you give me some suggestions on how to shake these sad feelings. I cannot wait to go back to work (because things will be normal again). I'm sorry if I'm rambling.

Detailed Answer:
Hello,

Thanks for writing to us.

No need to be sorry.I can understated your feelings.

From your narration I found following symptoms you are having currently:
worry,
feeling of being looser,
apprehension of losing parents,
not eating well,
loneliness.

These are some the symptoms of depression.

To diagnose depression detail history, physical examination and mental status examination needed by a psychiatrist in personal.

Core symptoms of the depression : low/sad mood and/or lack of interest in routine and pleasurable activities. One of them must be present for at least 15 days for diagnosis.

Other symptoms are hopelessness, worthlessness, excessive fatigue, increased or decreased sleep, increased or decreased appetite/weight, death wishes. Four of them should be present.

Either person feels above symptoms or someone around him notices these symptoms.

With these there should be some occupational or social disturbance in life.

If all of above criteria met, psychiatrist can reach to conclusion of diagnosis of depression.

With above information if you think there is possibility of depression, consult doctor/psychiatrist.

Let me know what do you feel regarding about these symptoms in your case.

I also want to know,"any specific social, economical, or relationship stress do you have right now? Do you have any physical problem?''

Hope I have answered your query,

Regards,
Dr.Chintan Solanki.

Thank you for the feedback Dr. Solanki,
I do have some of the symptoms you listed but they have not lasted for 15 days. Sometimes the feelings come and go but it's only been this last week that I've felt these symptoms. I watched a movie the other day and I really felt normal after. I'm a worrier in general so I tend to worry about really mundane things at times. I have no physical problems - overweight, high blood pressure (which I control with meds). I have the typical economical stresses. Is there some literature I can read to help me cope with my situation? Should I wait until 15 days before consulted a therapist?
Thank you again.

Brief Answer:
You can wait, read book ‘Feeling Good' by XXXXXXX burns

Detailed Answer:
Hello XXXXXXX

Thanks for follow up.

If your problem is not persistent, you can wait.

Continue regular sleep, meals and exercise.

You can search for a book,'Feeling Good' by XXXXXXX burns. It is a very good book for help in stress and depression.

For your economical stress as well as for your worrier nature you can visit psychologist or psychiatrist for counseling and psychotherapy if you wish. Or you can wait for 2 weeks to see you become absolutely fine or not.

Hope I have answered your query. If still any query there, I will be happy to answer. If not, you can close the discussion and if feasible give your feedback so I can improve my service. In future you can contact me directly if you wish with this link: http://bit.ly/drchintansolanki

Hello Dr.,
I checked into the book. There were a few of them. Was there one in particular that I should read?
I have made an appointment to see a therapist but (strangely) I don't want anybody to know.
I have been going to the gym the last two days (straight). I haven't been too hungry but I'm trying to eat a little.
I informed the psychiatric nurse of my worrying nature and stress and she informed me that the doctor I'm seeing can address these problems when we speak.
Thank you for being here. Just writing down my problems helped me. Thanks again.

Hello,

Thanks for writing back and sorry for delayed reply.

The book I suggested is particular. Check out this link: https://books.google.co.in/books?id=YYYY=feeling+good+by+david+burnsYYYY

It is good that you are going to see therapist. It is a good step. You will be helped in very positive manner.

For appetite do not starve yourself, but try to control diet in terms of calorie and fat.

Just narrate the problem to someone is healing sometime. You wrote here as well as informed nurse. This has helped you. Likewise discussing problems with therapist will provide more help.
