
Suggest Treatment For Aortic Tear And Hypertension

Question: I have recently experienced an aortic tear and am on doxazosin 4mg once a day to keep my blood pressure under control. I also have a postural drop in bp when i stand and bp rises through the day peaking late evening - from around 125 systolic sitting in the morning to 165 late evening. Any thoughts re the postural gap and the late evening peak much appreciated.

I read your medical history carefully and understood your concerns.
The postural gap and late evening peak is due to not-well treated hypertension.
If Ramipril gave you cough, then ARB-inhibitors are advised to start.
I advise to add diuretic drugs like HCTZ for example.
I do not advise to use doxazocin, but to switch to ARB-inhibitors and add diuretic first. If the bp will not be stabilized, then, can use doxazocin as last choice antihypertensive drug.
