Question: I have painful white blisters on the tip of my tongue on the under side. I am in good health, no fever, good dental health, etc.

Based on the image you uploaded and your history, Aphthous ulcers is the most likely diagnosis.

B COMPLEX VITAMINS and FOLIC ACID supplementation should be prescribed for a month. BONJELA OINTMENT containing cetalkonium chloride and choline salicyltae can be used locally for the pain and burning/irritation. Mouthwash such as enziclor or pyodine should be used 3 times a day to maintain adequate oral hygiene. Doxycycline course can be prescribed if the lesions persist despite the above measures. Tobacco chewing and smoking should be strictly avoided.
If the lesions still persist, then an oral brush biopsy may be indicated in consultation with your doctor.

What could have caused these so I can avoid in the future. Can I get the Bonjela
Ointment and the mouthwash over the counter without a prescription and where for instance? The only thing different recently was I took Aleve for my back.

Hello again,

Exact causes of Aphthous ulcers is not clear but correlation with some predisposing factors has been made in patients with recurrent aphthous ulcers.

Stress e.g in times of examination, immigration, marriage issues/divorce, financial worries etc
Trauma due to dental appliances or mucosa bites.
Food allergies like pomegranates, cola drinks , peanuts etc
Abrupt cessation of smoking is also related to Aphthous ulcer formation.
Sodium Lauryl sulphate present in some oral hygiene products can also lead to such ulceration.
Many other factors like B12 , iron, folate deficiencies and genetic predispositions have been associate with aphthous ulceration. NSAIDs like Aleve can lead to aphthous ulcers.

Bonjela and the mouthwash are available over the counter. The bonjela contents have been mentioned in the previous reply i.e cetalkonium chloride and choline salicylate. You may ask your pharmacist to give you an alternate brand if its not available. Somogel is a similar product that is equally effective.
