Suggest Treatment For Asymmetric Lymphatic Mass In Lingula Lymph Tissue
Question: I have a assymetric lymphatic mass in the lingular lymph tissue confirmed by ent. Also right sided parotid enlargement, facial swelling, and hoarseness, fatigue, night sweats and brain fog. Surgery is oct 2nd. Should it be sooner ans what are the possibilities. I took remicade and humira since 2003.

1. Lingual tonsils can be unequal in size in most of the cases and that's perfectly normal. It is considered pathological in the conditions such as base of the tongue malignancy, which is very unlikely in your case.
2. Symptoms other than this could be due to thyroid related issues or lymes disease or lymphomas (doubtful).
3. As you have not mentioned about the type of surgery you are supposed to undergo, it is very difficult for us to comment regarding the urgency of the procedure. If you could kindly revert back to us with more details about the present ailments and the name of the surgery planned, we can help you in a better way.

Yes a complete tonsil and adenoidecfomy- however my tonsils add gome- so he is just taking the lymph mass oit
On the rifjt. I cant remember if i told u my parotid is also enlarded on the right, with swelling, intermittent facial drop and motor and cognitive decline. Neurologist did spinal and just have elevated lymphs and neuron specific enolase. Neuro was think neurro endocrine tumor? I also have symptoms that align with carcinoid- flushing, sweats, nause, anorexia, malaise, dumpimg syndrome, low mag and K from dehydration. And drenching nighg sweats. Where would u go to get best neuroendocrine work up and eval?
Also, I have been admitted for encephalopathy and have had altered speech, facial droop, tongue deviation. Fine motor symptoms and cognitive deficits- getting lost, word finding just not my old self. And doing a lot of mistakes.

1. Tonsillectomy is done to rule out lymphomas. As adenoids would have already been regressed in your age, they may just take a small biopsy from the nasopharynx. After tonsillectomy you will have throat pain for a week. As the surgery is done to send the tonsils for biopsy to rule out lymphoma, you can actually wait till the 2nd of October.
2. CSF study shows inflammatory changes in the brain. It could be secondary to encephalopathy. Brain metastasis can be diagnosed with MRI of the brain.
3. Elevated neuron specific enolase is a marker of neuroendocrine tumor anywhere in the gut tissue. The best person to manage this is the medical gastro-enterologist. You can contact any of the below mentioned specialists near your place.
