Posted on Mon, 1 Aug 2016
Question: Hi, My wife is 11 weeks pregnant and yesterday she got the blood spot in the evening around 7 pm and late in the night around 3 am started getting severe back pain and bleeding ( like periods). We went to the emergency, they took the blood test and mid wife nurse checked her cervix and it was tight and close and later bleeding stopped or not much bleeding. She advised the ultrasound tomorrow as my wife BHCG Level reduced significantly which is now 4000. Can you please advise?

Brief Answer:
threatened miscarriage

Detailed Answer:
Hello,
thanks for the query to HCM,
This is a threatened miscarriage with possible placental separation. It is important that foetal viability should be confirmed asap and one cannot wait for another 24 hrs. You must visit ER asap and get the USG done sooner to check for foetal heart rate and any retroplacental clot. Because if the foetus is viable then the next mode of treatment shall be progesterone support in form of pills and injection and absolute bed rest till bleeding completely stops.
The decrease in beta hcg levels is not a good sign at all.
Please get a transvaginal ultrasound soon and review with reports. Do not wait until tomorrow.

Issue here is in Australia, no one works on weekend and there is no ultrasound facility available during weekend and we already been to medical emergency and they have suggested what i mentioned above. Is there any on the counter medicine can be taken to get some progestrone support pills or home remedy as it looks very difficult to get any one till tomorrow.

Brief Answer:
visit ER again for prescription

Detailed Answer:
Hello,
Progesterones are not available as an OTC med. They need to be prescribed by a physician. I feel you should visit ER again and ask the doctor to prescribe you the same ( micronised progesterone 200mg PER ORALLY once a day) for at least 2 days till you get your USG done.
