Hello XXXX, I understand your health concern.
Basically your problem is perimenopausal osteoporosis which is causing all these back pain symptoms.
Keep taking BHRT as advised as it will take 1-2 years of regular intake to have good effect on bones and helping in maintaining the mineral density.
Along with BHRT take a note of following points -
- take milk on regular basis at least twice a day making total of half kg intake a day
- take calcium supplements 1000 gms a day
- choose a calcium tablet which comes in combination with vitamin D
- visit a good physiotherapist and learn some back strengthening exercises which you can easily do at home (will have the best effect in shortest span)
Also there is alendronate infusion which is available in market and is given once an year intravenously and is very effective in maintaining bone mineral density but for this you will need to visit your gynecologist.
As regards dirty black discharge during periods there is nothing to worry as it is because of endometrial atrophy with the passage of age and thus very less tissue to disintegrate during periods and thus lesser blood flow.

Seeing the history of rupture uterus and adenomyosis I will suggest you to go for hysterectomy as scheduled which will definitely give you relief and also will free you from risk of cancer due to hormone therapy.
But this pain might not get fully cured because most likely it is due to osteopenia and disordered nerve growth after uterine rupture from this pain fiasco started.
But still hope is there with hysterectomy and after that continued hormone therapy and calcium supplementation will help you recover better.
Till then try physiotherapy and hot fomentation which will give you the fastest benefit.
Also keep a record of your blood pressure and if needed (in case suggested by your physician) start an antihypertensive so that you don't get side effects on your body caused by prolonged presence of high blood pressure.
