Brief Answer:
Metastasis seems to have occurred.

Detailed Answer:
Thanks for asking on HealthcareMagic.

I have gone carefully through your query. Hepatitis C can lead to hepatocellular carcinoma (liver cancer) which could manifest with the spots you mention. After local spread, this can spread via blood to distant organs by a process called metastatis. Metastasis usually occurs to lung and bones. It seems that you are referring to bone metastasis involving the lower vertebrae. That would lead to the back pain and other related problems.

Sorry to state bluntly, but if metastasis has already occurred not much is left to be done and it is only a matter of time. My suggestion would be to make the best of life within the short span that this person has.

Regards
