Suggest Treatment For Back Pain

Question: I had piercing back pain one month ago and took hydrocodone/acetaminophen left over from dental surgery. Pain completely gone after two days and lots of rest. Then dyspnea on my daily walks and low BP 80/50 and high BP 200/110 during the last month. Had EKG and it showed "cannot R/O anterior infarct", "possible lateral ischemia", "T-wave abnormality", "ventricular premature complexes" and "bradycardia." I didn't connect the back pain so didn't mention to cardiologist who scheduled Sestamibi treadmill stress test and echocardiogram for next Wednesday. After seeing on Internet that back pain can be a heart attack, should I change the treadmill to the kind where they give you an injection to speed up the heart to get a picture of the coronary arteries and occlusions? My cardiologist is out of town and won't be back before the test next Wednesday.

I read your query and understood your concerns.

Before giving you my opinion I would like to check your ecg image by my self. Can you please upload the ecg image for me?

Based on the data you provided your symptoms are typical for a patients with heart disease (including heart failure and heart attack). Ecg is an important examinations in the diagnosis of heart attack, this is why I would like to check it.

If you were my patient I would suggest to do cardiac echo and cardiac echo stress (this is done with dobitamine i.v injection to increase HR and by doing so cause myocardial ischemia) in stead of treadmill test.

The stress test echo is done directly after standard echo and it's of help in differentiating between exercise induced ischemia (heart attack) or heart failure.

Hi back,

After reviewing your ecg image I would like to say that it shows repolarisation abnormalities compatible with ischemic cardiomyopathy (i.e you have suffered previously from a heart attack).

However ecg doesn't show the time of infarction this is why you should do the above mentioned test.

Moreover I would recommend to do also the following examinations:
- Blood work including cardiac troponin ( it will show recent myocardial ischemia)
- Cardiac catheterisation (angiography)
