Suggest Treatment For Bigeminy
I would explain as follows:
Detailed Answer:
Hello!
Welcome and thank you for asking on HCM!
Regarding your concern, I would explain that when dealing with cardiac arrhythmia it is important considering two important issues:
1. the specific kind of arrhythmia
-Are you experiencing an atrial or a ventricular bigeminy?
-Do you have only isolated extrasystolic arrhythmia or there are also episodes of a more complex arrhythmia like SVT, atrial fibrillation or even ventricular tachycardia?
In case of an atrial arrhythmia, facing your normal cardiac structure (normal ECHO), it would be hardly a dangerous or life-threatening situation.
But when we are dealing with a complex ventricular arrhythmia (ex. VT) a more strict therapeutic decision should be taken.
2. The quantitative aspect of arrhythmia is important to consider as well.
Before judging about the clinical importance and underlying therapeutic options of a certain arrhythmia, we need to know how frequent this arrhythmia is appearing during at least a day-night period.
For both of the above reasons, it is necessary to review some specific medical tests:
-Holter monitoring (ambulatory 24 hours ECG test)
-thyroid function tests (to clarify whether your actual therapy has adequate dosage)
-blood electrolytes levels
-exercise cardiac stress test
-arterial blood gas analysis.
Only after concluding these results, we could think whether a therapeutic strategy is necessary to proceed.
You should discuss with your attending physician on the above issues.
If you perform any of these tests, you can upload them here at any time for me to review.
Hope to have been helpful!
Kind regards,
Dr. Iliri
