Suggest Treatment For Bladder And Kidney Infection

Question: Hello, doctor. I need guidance, please. Ive been having infection problems for a while now. What started out as a UTI is now something else that I can't pinpoint. Long story short, I was given antibiotics about 3 or 4 times for UTI (the first dose I didn't finish completely) which then developed into a yeast infection, which I was also medicated for. I was given Flaxistatin V (metranizadole and something else, can't remember) for itiching and burning in the vagina. I took all of this. Presently, my symptoms are divided in two and I'm wondering if they are part of the same problem. First, I feel pressure against my bladder, which leaves me feeling puffy and it's hard to walk, an urge to pee (but little or nothing comes out) and inguinal pain, as well as itchiness and jolts of heat in my thighs and joint pain in my knees. I also get lower back pain, whenever I press against my lower back, my bladder feels irritated. On the other hand, I continue to have a sensitive, itchy vulva and burning sometimes. I'm not sure but I think I might have bad odor too. No cheesy discharge, just some watery sticky white stuff. Could this be an STD? I would like to go and get myself tested for gonorrhea or clamydia. Could it be Inflamatory Pelvic Disease? I would like to know these answers ASAP before I consult my gynocologist and take my tests. She recommended a bladder ultrasound. I need help and feel somewhat depressed that this has not been diagnosed or solved....

I can understand your concern here.

Please do not take any stress, your problem required some investigations to rule out the cause.

I have gone through your history and before giving my opinion I would also suggest some investigation which are helpful in making a diagnosis.

As you are having frequent UTI, the chances of bladder and kidney infection are very common( due to back pressure of urine) and your first symptoms can be due to bladder and kidney infection.

To rule out whether it is due to infection of bladder or kidney, you have to go for ultrasound abdomen, urine culture and sensitivity, creatiniine and uric acid.( PID is also rule out in ultrasound)

Your vaginal itching and foul smelling discharge can be due to vaginal infection and it can be bacteria, yeast infection or STD.

So you should consult a Gynecologist and get yourself examined and take vaginal swab culture and sensitivity, STD testing to rule out the cause.

Are you diabetic?( because it can cause frequent uti and vaginal yeast infection).

Have you done unprotected sex?

You are taking oral pills or any other medication?

Thank you so much, doctor. I am not having pain while urinating....could I still have a UTI? I mean, it doesn´t burn or anything like that. It´s just pressure. And very little urine and swelling. If I give them a urine culture, should I ask them to look for a bacteria specifically or can they just determine whatever may be the cause here? I ask because I asked a microbiologist and she said that she could only determine it if it was a UTI. Not anything else.

Is the ultrasound supposed to be in my bladder or just my abdomen?

How do you take a uric acid and creatinine test?

Again, should I ask to look for a specific bacteria in my vaginal swab test or just any?

As far as I am aware, I am not diabetic. But my father is at risk. Should I be tested?

I am taking oral contraceptive pills. Microgynon, at the time I was infected, now I am on Qlaira, per GYN´s request. Which means that Ive had sex with my boyfriend without a condom.

Other symptoms which you have are also be suggestive of UTI.

In urine culture and vaginal culture they will find out the causative organism if it is there, so no need for particular organism culture is important.

You should go for ultrasound abdomen not bladder ultrasound.

A blood withdrawn for uric acid and creatinine and test done with blood serum.

Yes, you should get your sugar test done.

Yes with recurrent UTI it is suggested to change a birth control pills.

Yes you should get your STD testing done.

Excellent. Just one last question. I find that during times of stress, the discomfort (whether it be burning/itching or bladder pressure) increases. Is that normal? Also, may I contact you if I have further queries after my tests?

Hello, doctor. I do have one question.

I finally was able to get a swab test at around 10 in the morning which I took to a private lab to get cultivated. Problem is, I had to wait until around 3 pm to go to the lab. The swabs were placed in this gel in the tube which I assume was to keep intact. But I did leave it out in room temperature for around 4 hours (the last hour before going I was told to put in the fridge). Would this sample still be good to test? The lady at the lab told me so but I wanted to make sure. Should I go and get another swab done just in case? Please, all I want is to get proper results. I would like your advice.

As per my knowledge, vaginal swab for culture can be kept at room temperature for 24 hrs. before culture.

In your shell as you kept the sample at room temperature is fine, but then you refrigerate the sample, so I doubt about it.

One more thing is here if the transport media( gel) is dried up than please do not use that sample, it will give not give you proper result.

Hello, doctor. Got an update from the lab results...everything turned out negative (for syphillis, herpes, clamydia, gonorrhea, and HIV) and negative for the UTI. The vag swab did say that there were few positive bacillos shown...

Im not sure what to do now but to go back to my GYN and have her interpret the results. I am also pending on my ultrasound. Should I go to a urologist, by any chance? What do you recommend I do?

Im not sure how to proceed now
I should add that I still feel burning, soreness and itch in the vagina and also some bladder pressure. Inguinal, thigh pain has died down lately.

Please do not take any stress and feel happy as your test results are negative.

UTI is negative may be due to antibiotic treatment.

We can wait for ultrasound report and plan accordingly or you can consult a Gynaecologist for vaginal infection treatment.

You should drink a warm water, keep the area dry and clean, wear loose fitting undergarment, do protected sex, avoid perfumed products.
