Suggest Treatment For Blocked Artery, Diabetes And Cataract

Question: My husband had stents fitted in July 2013 after being almost completely blocked. He didn't have a heart attack but had suffered angina pain for a few years. He had two stents fitted side by side into the right coronary artery. Since then he has been advised to exercise and eat healthily but he doesn't do either. He goes everywhere by car and never exercisers and has a very bad diet. He weighs about 240 lbs and is 5 foot 8 inches tall. He has poorly controlled diabetes (type 2). He also has hypercalcemia glucoma the start of cataract and in addition bipolar. he is 67 years old.

Hello XXXXXXX Welcome to HCM.
I have read your question and understand your concerns.
Your husband is an obese Diabetic patient who has undergone angioplasty previous year. He must follow this instructions..

1.Regular daily brisk walking for 1 hour as it increases blood flow to the heart muscles, controls blood sugar levels also.

2.Avoid oily foods and milk products as eating oily and backed food items increases cholesterol levels and chances of other blockages will be high.
Sunflower oil or olive oil can be consume on limited quantity. Eat more fruits and green leafy vegetables. (Apple, orange, pomegranate can be eat in limited quantities in diabetes)

3. After angioplasty he must continue antipletlet agents (aspirin and prasugrel - blood thiners) for 1-2 years to prevent re blockedge of stents. One of the antipletlet drugs should be continued lifelong.

4.Do his FBS, PP2BS AND HBA1c report (3 monthly average blood sugar levels) at regular intervals, at least 3 monthly for adequate sugar control.

5.kindly also mention which medicines he is taking as I can guide him further.

6.After angioplasty we usually do trademill stress test after 6 months to see if a person is having any further problems or not and how the stent is working. So he should go for Trademill stress test and 2d echo (sonography of heart) on priority basis, so that his doctor can understand what is current heart condition.

7.Do his lipid profile, CBC, kidney profile and serum calcium levels and show it to your doctor for further guidance. You can show it here so that we can guide you further.

What's the prognosis if I can't get him to follow doctors orders? his medications are lithium paroxetine bisoprolol aspirin omeprazole and simvastatin.

Your husband is taking following tabs -
Tab aspirin for heart
Tab bisoprolol for BP and heart
Tab lithium and peroxitine for bipolar disorder
Tab simvastatin for high cholesterol
Tab omeprazole for acidity(which he can omit if he doesn't have acidity)

I couldn't find any diabetes medicines first of all. So please kindly reconfirm regarding that.

Secondly if your husband is not following doctors instructions about diet and exercise which I have also advised, at least take this all medicines regularly. It will also helps to reduce further problems.

Due to his bipolar disorder he might be resistant to follow doctors advice. So he should visit a good psychiatrist personally. It will help him in both problems.

At least you should give him diet which I have explained to you - fresh fruits and vegetable soup daily. Avoid Non vegetarian diet if possible.

Now coming to your question if still he will not follow doctor's instructions then he might have further heart disease, kidney problem, worsening of glaucoma. Medicines will do their work definitely to prevent complications and at least delay it. So it's not emergency to visit the doctor at present. Just explain him calmly about his problems and further complications and importance of visiting a good doctor at least twice a year.
