Question: Dear concerned specialist,

I was diagnosed with Severe Reflux Esophagitis as of my endoscopy.
my worst symptom is breathing difficulty, feel food and liquid coming back up.
My doctor gave me Omeprazole 20 mg to take. I have taken it for 3 weeks so far.
how long does it take this medication effect?
what can I do to swallow food easier without coming back in meantime?
is there any diet recommended?

Hi,

Thank you for your query.

1. I would recommend that you should request your doctor to add a prokinetic such as domperidone, levosulpiride or cinitapride. Mucaine gel or Gaviscon will help.

2. The medication is best taken in the morning, half an hour before breakfast, on an empty stomach. You can repeat the dose in the evening, if required.

3. The esophagitis may require 8-12 weeks of medication to control.

4. A bland diet, lifestyle changes and eating 2-3 hours before lying down will help. Avoid spicy, fried, oily food and food that causes discomfort.

5. Get a USG (Ultrasound) abdomen to rule out a hiatus (sliding) hernia and test for h.pylori infection.

Do you think drinking water after meal can help to reduce symptoms?

Do you see any benefit for me to take two doses of Omeprazole 20 ? and if yes, in what manner?

Can esophagitis or reflux causes breathing problem and how it should be addressed?

Avoid too much water after meals at night.

Hi,

Thank you for writing back.

1. A glass of water after meals is fine but avoid too many glasses of water after meals and especially late at night.

2. The ideal dose of Omeprazole is 20 mg twice a day. One dose will not cover the whole day. You are free to adjust the dose and timing that best suits you.

3. It is common to feel breathless with acid reflux. The acidity and the digestive enzymes that reflux into the throat cause irritation and inflammation. Mucaine Gel will help. After a few days of treatment, this unpleasant symptom will go away.
