I have gone through your medical history carefully and the photo you sent in carefully and understood your concern.
The red bumpy rash on your chin seems like cellulitis ( skin infection) from the photo.
At the beginning it could be started as a little rash after shaving. Then it might have grown bigger.
I was not able to see your cheeks. If they have bumpy rash as well, rosacea is to consider.
In both cases, antibiotics are advised to start orally. Locally can use alternating warm and cold compresses and antiseptic a( e.g. betaine). If still no improvement, topical antibiotic cream should be used.
