Suggest Treatment For Burning Sensation During Urination
Before I start, I'd like to say that I of course plan on visiting the doctor's office to get tested for HIV next week, I just wanted to ask for an opinion here as I live in Japan and would like to do so once in English. This will be a long explanation. I thank you in advance for your time and patience.
Exactly one month ago I had unprotected intercourse with a questionable individual (female). I have had chlamydia before in the past. My only symptom then was a burning sensation while urinating. When I went to the doctor, he said it "seemed" the lymph nodes in my groin were swollen but he wasn't sure, and I didn't really notice any difference. He took a urine sample and gave me antibiotics. When I went back after finishing the medicine, I learned that I had in fact had chlamydia.
My symptoms now are different. It doesn't burn when I urinate, and there isn't any discharge, but there is a slight dull pain that sometimes occurs inside of my urethra. Also, the lymph nodes of the right side of my groin are swollen, it aches to press on them, and I sometimes feel a bit of pain just when going about my day. It even feels as if it is causing a bit of stiffness in my upper leg and hip. I must also mention that I am a worrier: At about 2.5-3 weeks, I felt really bloated for about two days. I couldn't tell if I felt cramps or slight gas. This could have been from stress related to work and general life, which is relatively non-stop. Also, my knee began to hurt just today, but I am a drummer. The only symptom that is as clear as day is the swollen lymph node and perhaps the pain in the urethra. I haven't had any cold symptoms at all (today marks 4 weeks exactly), only everything I described above.
I'm wondering a a couple things. I am aware that HIV doesn't always have any symptoms and that they can be different from person to person, but based on the country I am in and the things described above, what do you think? Also, is it possible that while my chlamydia symptom last time was a burning sensation while urinating, my body could just be reacting differently this time? Also, I guess it could be other STDs as well?
Another thing that might be worth mentioning is that while I don't really get allergies, my body does tend to 'react' in its own way to seasonal changes. This time of year, as it's getting warm, I tend to feel a bit weak and since I have bad skin I get itchy and fuzzy sensations on certain areas of my body.
I think that is all I wanted to say. I appreciate your response. Thank you very much!

Unlikely to be acute HIV symptoms but please get tested
Detailed Answer:
Hi XXXX,
Thanks for writing in to us.
I have read through your query in detail.
Please find my observations below.
1. Lymphnode enlargement is a response by the body to fight any offending infection. Such an infection might be bacterial, fungal or viral. Usually acute lymphnode enlargement is due to bacterial infection and you are aware the last time you were treated for chlamydia.
2. Chlamydia is an infection that shows strange symptoms and in males only about 1 to 3 in 10 develop symptoms. Since you have a history of chlamydia and are again having swollen groin lymph nodes with slight dull pain in the urethra, it will be better to get tested for it also.
3. The two days when you felt bloated are unlikely to be symptoms of acute HIV infection. Should you have had an acute HIV attack then there is sudden onset severe flu symptoms with generalized weakness that does not improve in two days. Therefore in my opinion that was likely to be a gastritis related or episode of cramps and unlikely acute HIV syndrome.
4. Since you are a drummer, the knee pain can be explained due to that and especially if you have done long duration practice in the past few days. Your seasonal allergic symptoms are non contributory to your present symptoms.
5. There are many HIV tests offered in different hospitals. I suggest you please take the HIV fourth generation rapid test which tests for HIV 1 / 2 antibodies and p24 antigen. This test should be done at 30 days for accurate result.
6. You can also get tested for complete STD screen to make you feel safe. If your chlamydia test is negative and also STD tests are negative then you must get tested for non STD infections like non gonococcal urethritis.
7. Hoping the best for you and I am hopeful that your HIV test next week will turn out to be negative. Please always use condoms when having sexual intercourse with a partner of questionable sexual health.
Hope this answers your question. Please feel free to correct any oversight in my interpretation of your problems and discuss them in detail as per your requirements.
Hope your query is answered.
Do write back if you have any doubts.
Regards,
Dr.Vivek
