
Suggest Treatment For Callus On Heel And Black Discoloration Of Skin On Feet

Question: I have a callous on the heal. I sanded the dry skin and the next day the dry skin came off leaving black skin underneath. it is round and the size of a quarter. There is no pain, no fever, I walked 6 miles today, no redness and I feel 100%. But it looks terrible, what I imagine necrosis to be.

By way of background I am on blood thinners aspirin 160 mg and Effient (prasugrel) 10mg, as well aa metoprolol 50mg, Lisinopril 5 mg twice a day, Lipitor 60 mg and isosorbide mononitriate. I have very very low blood pressure for the past year since cardiac issues 90/45, pulse 62. Ejection fraction 40%. Also ultralow cholesterol at 88, with LDL of 23 and HDL of 46. I walk 6 miles per day, I am very fit, I never get breathless, I have excellent circulation and have no diabetes with normal blood sugar. I work 10 hours /day. I have never felt better or more energetic.

Hi.
This seems like a bruise. The trauma of sanding and prolonged walking are the likely factors that induced it.
The fact that you are already on blood thinners make you more prone to bruise even from slight trauma.
This should resolve on its own in about 10 days to 2 weeks.
You may take an Oral Vitamin C supplement once daily for faster resolution.

Thanks for your response. While I want to believe your explanation, which I agree is probably correct, what made you exclude melanoma. My concern from reading the literature is that: (1) it looks a lot like pictures of melanoma of the bottom of the foot that I have seen online. (2) the bottom of the foot is very difficult to see and it could have been there for a long time. (3) The fact that I feel well may not be relevant since melanoma only becomes deadly after it metastases. Was there anything about the picture that made you think that it was a bruise.

A melanoma might resemble a bruise initially.
The factors that I considered in favor of a bruise is that it showed up after sanding and there was probably no sign of it before Or else you might have noticed it. A melanoma at this site would have shown through the thick skin and it would'nt require a sanding to be visible.
The fact that you are on blood thinners makes it more likely that you would develop a bruise easily after trauma.
I gave you a time period of 2 weeks and asked you to take a Vitamin C supplement.
The bruise should show some resolution, partial or complete, by then and if it does'nt then we could consider melanoma.
