Brief Answer:
endodontic treatment

Detailed Answer:
Hello XXXXXXX and welcome to HCM.
I assume that you are taking about the third molar as last tooth.

First of all if you dont have any pain or sensitivity then there could a possibility that the tooth can be saved by a normal filling.

But to be assured whether the decay has gone too deep or not an xray of that tooth has to be taken.

In case of deep decay exposing the nerves Root canal can be done to save the tooth.
But in some cases root canal of third molar is very difficult or not possible.
I would advice you to consult an endodontist to look out for the possibility of root canal otherwise that tooth has to be removed if causing problem.

You can even send me the photograph of the xray so that i can give you a precise treatment plan.

Hope this answer helps.

Brief Answer:
deep cavity

Detailed Answer:
Hello and welcome back.

It really seems a deep cavity but still needs an xray because you are saying that you don't have any pain associated with it.

If i were your treating doctor i would first take an xray to see the extent of decay.if its not involving the pulp(nerves) then a simple filling will do otherwise removal of that tooth.

Thanks a lot Dr!

Got your point. That essentially means Root canal or capping is not recommonded.
1. if the pulp is not there as it seems coz there is no pain - filling should do the needful
2. if pulp is there, extraction should be desirable.

Brief Answer:
endodontic treatment

Detailed Answer:
You are right about the treatment but let me correct you with the concept of pulp.
Pulp lies deep inside the tooth and its very important for a tooth.
Some times due to cavity pulp is exposed and infected then it causes pain.

But in your case i don't think that the cavity has gone deep enough to expose the pulp thats why you don't have any pain.your pulp is intact and healthy and that is a good sign.

So if that is the case a simple filling will do otherwise extraction is better option.
I don't think that you need to go for rct and capping.

Take care.
