default
Posted on Mon, 23 Feb 2015
Question: Good afternoon. My 3 yr old daughter woke up today with a chesty cough which started last night and slightly blocked nose. She does not have any fever and no throat inflamation and is not feeling unwell, infact apart from the cough she is fine. I kept her away from nursery today.
There are a lot ofcolds and flu about during this rainy season and if i take her to the doctors, here in XXXXXXX the first thing is always antibiotics which i would like to avoid if unnecessary.
I have rubbed her chest with vapour rub and keeping her warmly dressed and some cough syrup and paedeatric med lemon which contains vitamin c and paracetamol. And some garlicky chicken soup for lunch.

There is no phlegm or temperature so do you think she might need antibiotics? And if so which ones are recommended?

Thank you,

XXXX

Brief Answer:
Clinical condition and investigation will help in making decision

Detailed Answer:
Hello XXXXXXX
One day history of cough without fever may be due to mild viral infection, some kind of bacterial infection( some bacteria can cause respiratory infection without fever i.e. mycoplasma), or due to allergy.
I want to know is there past history of repeated respiratory infection?
If yes then whether it is associated with increased respiratory rate or retraction of chest most of the times?
Does she needs frequent nebulisation?
Is there family history of asthma or any kind of allergy?
Answer of all these questions will help in making diagnosis of any allergic predisposition.
You have mentioned there is no fever then why are you giving paracetamol
I would suggest you to to closely observe the baby, continue the feeding and if there is increased respiratory rate or chest wall retraction, decreased oral acceptance consult your physician. Till then give some cough syrup containing antiallergic and bronchodilator by consulting a physician. If condition worsens investigation may be required to see whether there is any infection or not and even some antibiotic like azithromycin or amoxycillin & clavulunic acid may be needed but must be under physician guidence.
Regards
