Question: Hi,
Need opinion about my father's condition. Diabetic and hypertension since 30 years. He had 2 stents in a single artery in Jul 2015. Had constipation problems a month ago which got cured in hospital. He started getting body tremors few weeks ago and at times didn't eat food or medicine. Had psychological problems as well. Presently, he is in psychiatric rehab centre and being fed medicines and food through ryles tube since he feels that his throat is choking and he stopped food and medicines intake a week ago. His health is deteriorating. Now diagnosed with High Urea, 68 and creatinine 1.7.

From the history and investigations you have furnished it looks like your father is suffering from Acute kidney injury which is most likely because of dehydration. In the discharge summary it states that they were suspecting urinary tract infection whoever neither does the urine routine show pus cells nor has the culture grown any organism so it is unlikely to be UTI

Here is what you should do

1. Dehydration is the most likely cause for increase in creat. Make sure to hydrate him well with clear water
2. It is important to stop telma tablet as it can worsen the kidney injury
3. Repeat and keep a check on creat, repeat the creatinine after 1 week.
