Suggest Treatment For Chronic Diarrhea

Question: I have had uncontrolled diarrhea for about 2 years now. Had a colonoscopy last year. Everything was fine. I think that bread is one of the causes. Pretty much stopped eating that. I am diabetic (type 2) and I have hypothyroidism. The t3 and t4 not working at all. I usually don't have a pain in m stomach when I have diarrhea. It just comes out...by itself...and wherever I am. Embarrassing. Should I pursue the IBS-D?

Hello. Thanks for putting your query in hcm. I will suggest few tests like anti transtissue glutaminase antibody to see for wheat allergy as celiac disease is commonky associated with diabetes and also avoiding bread makes you feel better. Second test is endoscopy and duodenal biopsy to see for any malabsorption. Third test is stool examination to see for pH of the stool and reducing sugar and any fat loss suggesting carbohydrate malabsorption and fat malabsorption respectively. And fourth test is breath test to see for bacterial overgrowth which is also associated with diabetes and cause malabsorption. If all these tests are normal then it can be labelled as IBS D. And for I will suggest few medicines which I found very useful in my patients with IBS D. Antisecretory agent loperamide , may start with 2mg once a day and can be titrated as per response. Probiotic and tablet rifaximin which is gut specific antibiotic that is it acts only in intestine and is not absorbed systemically. However in any of hte above test is positive then it is to be treated accordingly. I hope I have answered your query and this will help you. Wish you a good health.
