Suggest Treatment For Chronic Gout

Question: hi my name is XXXX I am 43 years old I am pretty active 6' 225lbs and not much body fat except I have a little tummy . anyway have suffered from gout in the past in my toes and ankle. but usually its every 3 to 4 months between attacks I just recently went out on a walk got a pain in my ankle and of coarse figured that it was gout. so I started drinking a lot of water hoping to flush it out of my system and before I new it I started to feel my knee tighten up and of coarse it started to swell. so I went to our local walk in clinic and was prescribed indomethicin 3 times daily. I am a commercial fisherman and boat builder and have a deadline on a new boat and have been working everyday trying to meet our deadlines. I am on my third day of taking indomethacin and my nee swelling goes down at night but comes back during the day when I am working. this attack started 8 days ago the pain in the early evening when I lay down is incredibly bad then slowly gets better and by morning the pain is tolerable again but still can only bend my knee slightly. I was prescribed allopurinol in the past but never took it regularly. I am looking for a opinion on the matters. I am all ears.........

It can be treated with medication and diet restriction. For gout you may need medicine like zyloric, allopurinol and newer febuxostat you can start then according to blood uric acid level consulting your treating doctor. Febuxostat is best drug for maintenance therapy of gout. Food to be avoided in diet includes –meat and poultry. so the indomethacin that I am taking is it bad to take? and this febuxostat is it new this is the first I have heard of this. how long does this febuxostat take to accutually make a difference? I have heard that allopurinol can take months to take effect, is this true? what about cherry extract is that a wives tale? lots of questions.....

Brief Answer: It is good to consume cherry extract Detailed Answer: Hello sir, Taking indomethacin is not bad. It is an antiinflammatory drug and used to control gouty attacks or flares. After this flare is over, you may consider taking febuxostat after discussing your doctor.It may be beneficial to continue prophylactic therapy with febuxostat for up to six months. Some studies suggest that cherry intake is associated with a lower risk of gout attacks. So it is good to consume cherry extract.

so I just started the allopurinol in conjunction with the indomethacin day four and my gout has improved in my left knee but now it is flaring up in my right big toe. should I not be taking the allopurinol while I have an existing attack? should I stop until attack subsides?

Brief Answer: Continue taking allopurinol. Detailed Answer: Allopurinol can cause a flare-up of gouty arthritis during initial therapy. If you do develop a flare-up of gout while taking allopurinol, continue the medication while your doctor prescribes additional treatment to settle the attack.
