Suggest Treatment For Chronic Hemorrhoid And Complications With Defecation

Question: Hi i am 34 years old male, i am facing piles/hemorrhoid since last 15 years or so. And it's really frustrating that you can't tell anyone about your problem, so from last 1 week i am facing too much pain and can't do my morning business normally. In my initial years i was taking laxative for bowl movement, but down the line it stopped working after some years. So i searched internet for the cure for piles and read at many places that you have to push back the hemorrhoid by fingers, so while doing that i started moving my finger inside and it lead to empty my bowl. So now my brain has developed habit for this and can't poop without doing this, it's too hard to relax internal and external sphincter muscle and i really can't have my normal life, and i hope being a doctor you will take it as a secret confession and consultation, so that i can live my life normally. Please give some medication oral/tropical so that it will lessen the pain and sourness, I am attaching the images also so that you can help me better. I have already used PILEX OINTMENT and AMROID OINTMENT but no help.

Hi,

It clearly shows that you have Prolapsed piles.
It comes under Stage 4.
I am sorry to say that there is no Medical treatment for 4th Grade piles.
You have to undergo Surgery.
In few people, the Pile will become very painful due to Thrombosis(Blood clot)in it.
In some selected hospitals, advanced procedure like STAPLER HAEMORRHOIDECTOMY is done. The results are very good with this procedure.

Till then, you can use ANO METROGYL Cream.
Please take plenty of liquids. Eat fresh fruits, green leafy vegetables.
Nonvegetarian food, Alcohol , Spicy food should be stopped if at all you are taking them.
If your stool is soft, complaint of Pain and discomfort will be less.
But, for your stage, please consult a XXXXXXX Surgical Gastroenterologist, get examined clinically and proceed further with STAPLER HAEMORRHOIDECTOMY.
Nothing to worry.
There will be no pain as the entire procedure is done under Anesthesia.

Thanks for the reply doc, so what will be approx cost for the complete procedure in general, and as i mentioned earlier that i am only able to empty my bowl by fingering, so how can i re learn the natural was as i have tried many times but i can't, all muscle get more tense and hence only option left with the finger, and it's really pathetic and sad.

Hi,

First of all, you should not get tensed while passing Stool.
You should be relaxed.
After Surgery, there will be no need of your finger to evacuate the stool.
If high fiber diet is taken, there will be no problem.
Regarding Cost of the procedure, it depends upon the Hospital and the Surgeon.
