Posted on Sat, 1 Aug 2015
Question: I am a 24 year old female from XXXXXXX XXXXXXX I have been having a constant pain and burning sensation in my vagina/vulva for 1 year and 6 months. At some part of the day the pain is ignorable,at some other times of the day its worst. It aches when I sit with a little force or when i forcefully throw water on my vagina for cleaning. I can't wear tight clothes as it would aches more. I completely stopped wearing jeans. I have noticed that i have white discharge every day. But I consulted 2 to 3 gynaecologists and all of them said its quite normal.Discharge has always been white or clear. But it would be very sticky too. On some days discharge is quite thick too. None of the doctors have been able to find a solutions for this. This is quite embarassing that when ever I am sexually excited I get white discharge. Even the reasons like reading a romantic novel gets me sexually excited and i think therefore causes this white discharge. I am not sure about this. I am a virgin and hence none of the gynaecologists whom I consulted couldn't examine me furthur. They didn't find any thing abnormal while they examined. I am hoping that u would be able to provide some insights into this. Could u please tell me if I being sexually excited impacts my body and if that can be the root cause behind this? Could u also give me solutions on how to deal with this problem.

Brief Answer:
vulvodynia

Detailed Answer:
Hello.
Thanks for the query to hcm.
It appears from the history that you are suffering from vulvodynia. Tjis is chronic vulval or vaginal inlet pain without any obvious organic cause especially seen in young women. The vestibule may burning itch pain and irritate. This can make your life sexually socially and even daily routines can be hampered.
As you have white discharge this could be a fungal irritation as well so a course of oral antifungals shall not harm.

2. Take tab fluoxetine 1mg daily for 10 days .this shall help with the nerve irtitations locally and shall help you.
you may review with response.
Also wear loose clothes, use ice packs locally, avoid soaps and detergents to wash locally .
regards

Hi,Thanks for the quick reply.
But my doctor has prescribed me medicines for subcutaneous nerve damage already. I am taking Gabantin for past 3 weeks. But no improvement has been seen yet.
And one year before i had taken a course of oral antifungals for 1 week. But it didn't help at all. So can I rule out the possibility of fungal irritation
And doctor could you please tell me,if having white discharge almost everytime is normal?
If I take this medicine fluoxetine would it have any side effect?

Detailed Answer:
If you are already on gabantin for last 3 years then it would be better to graduate to fluoxetine. The white discharge if without itching and thin homogenous then its physiological.

Hello,
sorry that was a typing mistake.
Well , fluoxetine has a better response profile and safety profile. It would be ideal if you would switch to fluoxetine 1mg for next 10 days and watch for the response.
