Suggest Treatment For Corneal Ulcer

Question: Hello, I have had a problem with my left eye for 2 weeks. It started after removing a contact lens one night before bed, I had pain immediately after removal. It was sore but I went to sleep and there was no pain in the morning so I carried on as usual, wearing my contacts for the day. Two days later, I noticed that there was some discomfort so I removed my contacts and let my eyes have a rest. 2 days later the pain was still there and has worsened, my eye felt dry and was sensitive to light. It also felt like I had some pressure in my eye. There was no redness or discharge. The following morning I went to see an Ophthalmologist and was prescribed Azyter as well as Frakidex. I was told I had conjunctivitis. I should also add that I am currently in West XXXXXXX I arrived only a few days after the first incident with removing my contact lens. I have had a corneal ulcer as well about 6 years ago and I was concerned that I might have that again but due to language barriers, I could not explain my history to the doctor, as he only spoke french. So I completed the Azyter and was told to use the Frakidex ointment for 2 weeks. I am on my 10th day of treatment and it is only today that I have noticed a small area of pus on the bottom of my cornea. I had rinsed it out and it seemed to flush out but now it is back and its looking more like the corneal ulcer I had in the past. Please help me, I am in a third world country and I am not confident at all that my eye will be treated properly. I have attached a picture of the affected eye, showing the white spot. Its been sore today and sensitive to light. I really thought it was just an infection, I have used my medication prescribed correctly and have not used my contacts since the day before my doctor's visit

My dear,
Thanks for sending your query to us.

This is a corneal ulcer.It looks like a fungal ulcer to me,Though I in my practice put such cases to scraping and see the slide under microscope to confirm the diagnosis then and there.

Stop Frakidex immediately and as a rule do not put combination with Dexamethasone or Prednisone in your eyes ever with contact lenses.. This is a lethal combination of small injury by lenses and steroid application causes non healing ulcer or fungal infection.In countries like XXXXXXX the vial is labeled by court's order that preparation can cause fungal infection.

What you have to do is to continue with Antibiotic drops.I would be happy to change the antibiotic as you have used Azyter for 10 days now.In case if you could get Moxifloxcin or Gentamycin eye drops ,then I will be happier.These are to be put in 6 times day,one drop each.

What we need is Natamycin . This eye drop is not very easy to procure and one has to search for this,it's usually is a milky suspension and has to be put in one hourly when you are awake.
The treatment is to continue for about 2 weeks and you should get relief in about 4 days time.
