Suggest Treatment For Coronary Heart Disease Other Than Surgery
Question: Hi doctor, my father had undergone CAG on XXXXXXX 12 which revealed TVD and left RA stenosis. Doctor had done the stent to left RA (completed on 13/6) and suggested to go for CABG which was refused by my father. Doctors identified three blocks(90%,75%,50%) through angiogram. My father is 67 years old and he is having diabetics and high BP problems and is scared to go for surgery. Is it advisable to go for open heart surgery or any medication available to treat this problem?? Please suggest.

Detailed Answer:
Dear Sir
There are three modalities of treatment for coronary heart disease. These are medical management , angioplasty and open heart surgery. The choice depends upon symptoms, presence of diabetes, presence of loss of heart pumping, coronary lesions, patients choice and comorbidities.
Three vessel disease in a diabetic is usually treated with open heart surgery.
I would be able to throw more light on this if you can tell me the exact site of blockade in these vessels, his ECHO report and his symptoms.
All three modalities are appropriate in indicated scenarios.
Feel free to discuss further.
Sincerely

Thanks for the reply doctor
He got heart attack twice in 2003 and 2004 (which was treated only through medication without any surgery or angiogram) and then, he got chest pain twice, one in the month of april 2014 and other in may 2014. So, we went to a cardiologist for check-up. He took ECG and then did Coronary Angiogram. Here are the details which we understood from the doctor about blockages.
one vessel in the left side is having 90% blockage and the bypass vessel to it is having 75% and in the right side vessel , there is a blockage of 50%.
During angiogram, they identified a block in kidney and they performed Renal angioplasty for kidney block and cleared it and for Heart blocks, they said it is not possibe to go for angioplasty(stents). Please suggest for any alternative of open heart surgery for my father.

Dear sir
It would be best If you can upload the angiogram and ECHO reports.
If the lesions are not suitable for angioplasty, then only medical management with or without open heart surgery is the option. There are definitely certain advantages of surgery over medical therapy in certain scenarios. I request you to upload the two reports for further clarification.
