I appreciate your concern

Based on your description it appears to be a lower respiratory tract infection. Bacterial etiology if suspected should be ruled out based on history and physical examination findings primarily via auscultation of the chest.

X ray chest, complete blood count, ESR, CRP should be advised to reach a conclusion. Blood cultures should also be sent. Decision on antibiotics choice , bronchodilators and steroids can be taken after the investigations are available.
